Insurance

IMG_2509Our mission is to provide specialty physical therapy services to both men and women who suffer from pelvic dysfunction, functional limitations after cancer treatment and musculoskeletal conditions relating to pregnancy and post-partum. We have a comfortable environment that fosters healing in an integrative practice.

We have elected to no longer be a preferred provider for most insurance companies.  Instead, we provide physical therapy on a “fee at time of service” basis. By removing ourselves from a preferred provider/contracted status with the insurance companies, we do not have to limit the time or quality of treatment we provide because of insurance company restrictions. Nor do we have to elevate our rates to pay for billing services.  Additionally, we have the flexibility to assist patients in financial need.

We are in network with Blue Cross Blue Shield and Tricare.

Prior to your first scheduled appointment, we will call your insurance company to verify your physical therapy benefits.   You may also want to call them to completely understand your insurance plan. Below, please find an insurance company worksheet to help you ask the insurance company the right questions about your physical therapy benefits.  At the time of service and payment, we will submit the claim to your insurance company for their consideration of reimbursement to you.  The amount of reimbursement you receive will vary according to the terms of your insurance policy.  Some companies may reimburse you at 80%, some at 60%, some at 40%, and some may not reimburse you at all.  We cannot make guarantees or estimates regarding what reimbursement your plan allows.

We accept cash, check or Visa/MasterCard at the time of service.  Our fees are based on time spent with you and the treatments performed during your appointment. The fee ranges are as follows:

$ 150 for initial evaluation/treatment

$ 125 for 60 minute appointments

$ 75 for cancellations with less than 24 hours notice

Please click the link below to download & print out the Insurance Benefits Worksheet. Again this will help you ask the insurance company the right questions about your physical therapy benefits.
